Community Transition Services Resources
Michigan’s Community Transition Services (CTS) Program provides services to help people in nursing homes or other institutional settings return to the community. Transition Agencies across the state provide CTS to help people overcome barriers to leaving the nursing home – barriers like finding housing, security deposits, home modifications, and personal care – so the person can return home or to the community.
How do I get access to services?

Who qualifies for CTS?
CTS participants must be enrolled in Medicaid or be very close to approval for Medicaid. There are also some medical factors you must meet. For more detailed information, contact us at 1-888-746-6456
Housing and security deposits
Household needs such as furniture, window coverings, kitchen items, and linens.
Health and safety needs, such as pest removal, allergen control, and cleaning services.
Assistance with utility service initiation fees, including telephone, electricity, heating, and water.
Personal care services and help with activities of daily living
Non-medical Transportation
Home modifications such as ramps and grab bars; widening of doorways; modifying bathrooms and kitchens to make them more accessible
Various other transition supports and services

Mortgage payments
Unreasonable or unnecessary expenses
Food
On-going utility charges
Household appliances/items that are only for diversion/recreation
Expenses not identified in the person-centered plan
Expenses the person could meet or that some other resource will cover (including if they are willing to use an MI ABLE account for an expense that is allowable under MI ABLE)
What should I do if I get turned down for CTS?
You have the right to file an appeal if the transition agency turns you down for transition services, denies a specific item or request, or provides limited approval for an item or request you’ve made.
The agency must provide you with written notice, and you can file an appeal request with the State of Michigan within 90 days of the date on the notice.
